A Unique Operation Was Performed at the Givi Zhvania Pediatric Academic Clinic of Tbilisi State Medical University

As a result of the most difficult operation performed at the Givi Zhvania Pediatric Academic Clinic of Tbilisi State Medical University, the doctors managed to save life of a year and four months patient. This was the first surgery of similar technique and complexity in Georgia. The patient was not able to eat or drink for five days and suffered from cough. The doctors found an alien body below the first physiological narrowing of the esophagus. Hospitalization of the patient was risky. Professor Merab Buadze (Head of TSMU Pediatric Surgery Department) operated the patient and removed a 20 mm lithium element which was firmly fixed in the esophageal wall. According to the contemporary guidelines 20 mm lithium element should be resected during two hours. In this case the resection process was too late which was a precondition for later complications. After removing the alien body from the esophageal wall, a deep chemical burn was left with black dendritic fibrin masses. Endoscopic radiological examination with water-soluble contrast confirmed perforation of the esophagus posterior wall and existence of tracheoesophageal fistula without inflammation of the mediastinum. With the help of telemedicine and doctors from Rambam Academic Hospital (Israel) and Chicago Pediatric Clinic (USA) treatment plan was elaborated and planned. After increase mass of tracheobronchial exudates, surgeons: Ivane Tsuleiskiri, Assoc. Prof. Avtandil Kutubidze and Nick Buadze resected perforated parts of the esophagus and trachea and made unique reconstructive operation. The third gastrostomy operation was performed later. Intensivists, Dodo Gvelesiani and Ana Iakobashvili also anesthesiologists, Nino Sherozia and Goga Ioramashvili and all medical staff of the clinic made a significant contribution to save the patient.
